How can an Advocate Help You

Referrals

MAKING A REFERRAL

We take referrals from:
• Individuals directly.
• From their family or friends.
• From other services / agencies who might already be involved with them.

You can contact us by telephone, fax, email or in writing. We will need to take some details, so if you are contacting us on behalf of someone else please ensure you have their permission to pass on their information to us.

To access advocacy support you must be:
• Resident in Sandwell.
• Have an identified health and social care need.
• Have an identified advocacy need.

Sandwell Advocacy will work with individuals to develop an appropriate and relevant advocacy ‘pathway’ that clearly sets out what their issues and needs are, and then develops a mutually agreed action plan of how we can provide the most effective support.
